If your child suffers from seizures, please contact the school nurse with the following information and ensure that the Seizure Care Plan is completed by your physician.

If prescription medication is needed during the school day, please call the nurse to discuss.

What the school nurse needs to know if your student(s) experience seizures: 

• What type of seizures does your child suffer from?
• How frequently does your child have seizures?
• What behaviors does your child typically display during or after a seizure?
• When was your child's last seizure?
• What medications is your child currently utilizing for control of their seizures?
