Please ensure that we are aware of all your child's allergies. Please have the appropriate form completed by your licensed prescriber, including their signature as well as the parent/guardian signature. Medications should be brought to school by the parent/guardian in the original container with a pharmacy label.

By Law, all school children are served milk.  A Doctor’s Note is Required for:
